The Supply Capture Module is a customisable web based survey system used to:

Then contact health facility management and request they complete the survey via email. Then health facility administrators can enter information on current and planned health services provided in their facility.
Supply Capture administrators can then manage and analyse survey responses. The system is designed to continue to capture planned or current changes in health facility services at regular intervals in the future.
